The Application Form is for all camp workers - including Family Camp. This includes a Pastor's recommendation section to be completed by the Pastor of their home church. Take special caution to completely fill out the form. Note the list of guidelines and the page the workers are to keep for their reference. This Health Form is for all workers of Family Camp, and all campers and workers of Senior and Junior High Youth Camps, and Children's PT and Kaleidoscope Camps. This is mandated, by the State of New York, to be completed by anyone under 26 years old or they will not be able to participate in a camper or worker capacity. (Please copy this form as two separate pages for nurse’s administrative purposes – not on the back of any other forms.
